Who needs a Form VAF5?
A person who wants to visit the UK as a family member of somebody who is a member of the national European Economics Area (the list of the counties is indicated on the first page of this form) must file the online Form VAF5 EEA Family Permit.
What is Form VAF5 for?
The UK Border Agency examines all information on this form and may take a decision on applicant’s request for a permission. You should note that the Border Agency reserves the right to make a decision on your application without inviting you to an interview. There are penalties for giving false information or false documents — a bearer will be forbidden to visit the United Kingdom for the next ten years -- so an applicant should be careful filling out the document to avoid such consequences.
Is Form VAF5 accompanied by other forms?
An applicant should submit all the relevant original documents that they want the Entry Clearance Officer to see during consideration of this application. Your passport style photograph should be attached as well to the UK Border Agency Family Permit.
